To better care for its members and help them understand their own physical capabilities, the Boy Scouts of America recommends that everyone participating in a Scouting event have an annual medical evaluation by a licensed health-care provider – a physician (MD or DO), nurse practitioner, or physician assistant. Submitting your medical information helps confirm you meet the minimum standards for various activities. Unit leaders are required to protect the privacy of all participants by keeping medical information confidential.

Part A and Part B must be completed at least annually by all Scouting event participants. These sections cover health history, parental/guardian informed consent and release, and a talent release statement, and are to be completed by the participant and parent/guardian.
Part C is the physical exam required for:
- Any event exceeding 72 consecutive hours
- All high-adventure base participants
- Activities that are strenuous or physically demanding (service projects or work weekends may qualify)
Part C must be completed and signed by a licensed health-care provider – a physician (MD or DO), nurse practitioner, or physician assistant. Height and weight limits must be strictly followed when an event will take the unit more than 30 minutes from an emergency vehicle or accessible roadway, or when the program requires it, such as backpacking trips, high-adventure activities, or conservation projects in remote areas.
